Thank you for choosing ACCEL Physiotherapy and Sport Performance Centre for your care.
Please download & then complete the following Intake Form and bring with you to your first appointment (you will be able to save the form once you’ve completed it).

Intake Form

For your first appointment, please bring the following with you:

a) comfortable shorts for a lower body injury and a sleeveless shirt with no collar for an upper body injury,

b) insurance card (if applicable), and

c) referral (if you have one).

We direct bill many insurance companies including:

Blue Cross (including Veterans Affairs Canada, Canadian Forces, RCMP)
Manulife
Great West Life

Cowans Insurance
Johnson’s
Chamber of Commerce
Co-operators
Sunlife
NS Workers Compensation Board (WCB)
all MVA companies

We have two locations, 3711 Joseph Howe Drive, Halifax and 137 Venture Run (Suite 101), Dartmouth (at the Wright and Burnside Business Campus).
